School Form 8 - Learner's Basic Health and Nutrition Report (SF8)
At the beginning of the school year, with the assistance of the School Nurse (if any), adviser/MAPEH teacher will conduct actual measurements of height and weight each to calculate the Body Mass Index (BMI) value and weight status. The procedure is repeated as deemed necessary to measure improvement before the end of school ye replaces all other school forms on nutritional status with the same data elements re BMI.
